Gateways Transitional Planning Coordinator

SALARY: $1,924.00 Biweekly

JOB FUNCTIONS: Responsible for case management and discharge planning for the incarcerated Reintegration & Recovery clients returning to the community. Responsible for transportation, supervision and coordinating completion of assigned work detail for Gateways inmates to varied jobsites. 
• Performs case management utilizing intervention, therapy, rehabilitation, support services and continuing care strategies to the degree indicated by specific, goal-focused, measurable, attainable, realistic and time-limited client assessments and needs.
• Adheres to and ensures OBH client enrollment and case management compliance, including completing all administrative documentation requirements, assessment and evaluation completion, intake summaries, admission summaries, individualized client service plans, treatment notes, client contact notes and discharge summaries.
• Assists with client intakes, assessment screenings, data entry and admission documentation.
• Instructs and conducts Transitional Planning group educational classes on various topics.    
• Posts release contact, monitoring and documentation for aftercare assistance and transitional planning of clients reintegrating to community with Reintegration and Recovery participants.
• Assesses and assists clients with discharge and aftercare planning for housing, employment, medical, education and public services.
• Interviews, refers, follows up and completes case management discharges for the prereleased Reintegration and Recovery participants. • Monitors transitioned client compliance with community collaborations, criminal justice mandates, sobriety, independent living and employment.
• Accountable for all inmate workers, including driving inmates in a 14 passenger van to and from work sites, safety and security of inmate workers on assigned crew, notification for absconded inmate(s) from work crew, and overseeing of work detail(s) to ensure inmates do not steal, damage and/or remove/use any items that do not belong to the inmate(s). 
• Supervises and trains inmates on all equipment necessary to complete work assignment and use of supplies and procedures to care for and use equipment. Inspects all areas of responsibilities for quality and acceptable completion of jobsite responsibilities.
• Responds to any security or emergency situations involving inmate workers.              
• Documents any incident involving damage to equipment, property, transport vehicle or Gateways worker(s).
• Performs other duties as required. 
QUALIFICATIONS:
• High School diploma or equivalent education required.
• Ability to pass Jurisprudence Exam and register on the Colorado DORA psychotherapist database within three (3) months of employment.
• Prior law enforcement or detentions experience preferred.
• Ability to provide training on janitorial and worksite equipment.
• Proficiency in Microsoft Office.
• Must possess ability to exercise discretion and maintain high levels of confidentiality.
• Maintain regular and punctual attendance.
• An equivalent combination of related education and experience may be substituted for the education and experience requirements above.
• Must pass background investigation, motor vehicle record check, physical examination, drug screen and truth verification examination. 
LICENSES/CERTIFICATES:
• Must be listed on the Registered Psychotherapist database for the Department of Regulatory Agencies in Colorado within three (3) months of employment date.
• Must possess and maintain a valid Colorado Driver's License.
 WORK CONDITIONS:
May encounter some hazards to personal safety through contact with potentially violent inmates; work may be performed outdoors and require lifting, stooping, kneeling and reaching. May be exposed to variable weather conditions due to multiple work sites, including outdoor work sites. May be required to work variable hours, including weekends, evenings, and holidays as needed. Ability to safely lift up to 25 pounds.
