DESCRIPTION OF DUTIES/ESSENTIAL FUNCTIONS:
Under general supervision of the Unit Administrator and clinical direction of the Chief of Social Work, perfonns extensive casework in the role of Treatment Coordinator for patients in the rehabilitation program. Conducts individual and group therapy on behalf of the patients. Reviews social histories of admission; verifies social assessments are completed. Interviews assigned patients relatives and appropriate collateral resources Serves as a member of a multi-disciplinary treatment team, develops, designs, implements and evaluates treatment plans. Serves as a member of various Perfomance Improvement Teams.
MINIMUM REQUIREMENTS
Requires a master's degree in social work from a recognized college or university supplemented by one year of professional social work experience in the evaluation and treatment/habilitation of mentally ill, developmentally disabled or other disabled persons. Requires ability to develop and implement individualized treatment plans for forensic and behavior management patients in a maximum security setting. This class is included as an Upward Mobility Program credential title.

Anticipated Starting Salary Range – $59,028- $88,968
