Fatherhood Case Manager
The Family Tree, Information, Education and Counseling Center in Lafayette, LA seeks Bachelors Level Fatherhood Case Managers for its nationally recognized Healthy Start Program. Due to a five-year extension grant from the U.S. Department of Health and Human Service, services have been extended to include Evangeline, Acadia, Lafayette, Iberia, St. Martin, St. Landry and Vermilion Parishes. One position is open for experienced and passionate men with the ability to service clients in Lafayette and surrounding parishes. Healthy Start has a history of reducing infant mortality through strength based and home based approaches. The Family Tree seeks individuals with a strong initiative and a proven record of achieving outcomes with clients. You will receive excellent training and development, including monthly training, supervision, and peer support.
The case manager will provide home visitation, case management, and health education to clients of The Family Tree Healthy Start Program. Case management services will include early childhood development, effective parenting skills, and appropriate childcare. Services will be delivered in accordance with program time line policies. Responsibilities also include screening eligible families for risk factors, connecting enrolled families to a medical home, monitoring child health and referring families to community resources in order to meet their needs. The case manager will operate cooperatively as part of a multi-disciplinary team. Perform other duties as necessary, including but not limited to; recruit new clients in the program by doing individual and team outreach, create new linkages with community based organizations through the case management process, participate in weekly supervision, monthly staffing, bi-monthly case conferences and consortia meetings and participate in training sessions as recommended by the Supervisor and Director.
