Outreach Specialist - Families FIRST

Terros Health is pleased to share an exciting and challenging opportunity for an Outreach Specialist Families FIRST. 
 
This individual will provide outreach assistance to families in need of substance abuse services.  Working as part of a team for the Terros Health’s Arizona Families FIRST program at our Central location, this individual will impact the lives of families trying to overcome the barrier of substance abuse in order to reach the outcomes of permanency for children, family reunification and self-sufficiency.  The individual will need to be professional, friendly, a self-starter, organized, and compassionate.

Responsibilities
This individual will be responsible for documenting all outreach efforts in a professional, efficient, and timely manner. Coordinating treatment efforts with referring agencies and AFF treatment teams. They will Provide supportive services and resources, facilitate teamwork within the organization, and participate in program status and evaluation procedures.
Ability to work in fast-paced outpatient substance abuse program.
Responsible for the monitoring of outreach and treatment services for the Families F.I.R.S.T. program including coordination with the outreach team: as well as other Families F.I.R.S.T. treatment teams. 
Ensures that families referred to the Families F.I.R.S.T. program receive timely and effective outreach procedures including mobile outreach contact, assessment scheduling; and transition to treatment through coordination, monitoring and reporting activities.
Completes a Recovery Coach request packet in client's Electronic Health Record (EHR)whenever possible to engage clients who are resistant or are unsure of their interest in participating in the AFF program.
This position will be located within the TERROS Central office.

Qualifications
Education/Experience:
GED/High school diploma with a minimum of 3 years of behavioral health (BH) experience or an Associate’s degree in the BH field, or
Bachelor’s degree in BH field, or Bachelor’s degree in non-behavioral health field with 1 year minimum of BH experience            
Highly Preferred - Bilingual (differential pay available)
Skill sets required for success in this position include organization, prioritization, project management, the ability to manage multiple priorities, and excellent attention to detail and time management skills.
Working knowledge of the child welfare/child safety system.
Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment. 
Ability to pass the Central registry
Proficiency using the MS office suite.
Must have or obtain a valid Arizona Driver License, be 21 years of age with a minimum 3 years driving experience. Must meet requirements of Terros driving policy.
